Please arrive to your appointment a few minutes early so that we can have you fill out the appropriate paperwork for insurance purposes and your patient records here at our office. We strive to take care of our patients and will try to make the process as turnkey as possible for you every step of the way.

What to Bring to Your Appointment
We look forward to seeing you here for your next appointment. In order to best serve you, we request that you bring the following items to your appointment:
- Primary and secondary medical insurance cards. Please know in advance which one is which – we do not have a way to look this up for you. If filed incorrectly, you will be considered a private-pay patient.
- Government-issued photo ID card
- If the appointment is for a minor with a guardian, legal documents showing proof of guardianship
- List of current medications being taken
